Second Payment (50% of total)

Tier 4 —  Supported Cost: If significant financial constraints make the full cost prohibitive.

Payment Requirements: 

Due once the date you plan to take the medication is confirmed with us. This payment should be received prior to your traveling to Vermont to take the medication. 

Services Included with Second Payment: 

• We keep the medical team (doctors and hospice) informed of logistical plans related to your plans to take the medication. 

• Together we identify an appropriate and safe location to take the medication.

• We continue to work with you to coordinate funeral arrangements. 

• We meet with you and your family, friends and caregivers the day before you plan to take the medication to review the standard Vermont Medication Instructions; together we create a plan for that day.

• We provide a trusted courier to pick up the medication from the pharmacist and deliver it to the location where you’ll take the medication.

• On the day you take the medication, two members of our team are present: we provide compassionate, practical care. At the family’s request, we mix the medication and monitor the process; after death we notify hospice and the funeral home, which initiates the death certificate process.   

• Coordination with hospice and funeral home for after death care.

• We remain present with family, friends and caregivers until the process is complete.  

• We provide resources about free grief support groups.
